Annual Wellness Visit

If you have Medicare Part B (Medical Insurance), you are required to go to  annual wellness visits to develop or update a personalized prevention plan. Wellness exams differ from physical exams, which tend to be more extensive.


In most cases there is no cost associated with an annual wellness visit because it is considered preventative care.

However, you may have to pay coinsurance, and the Part B deductible may apply if:

  • Your doctor or other health care provider performs additional tests or services during the same visit.
  • These additional tests or services aren't covered under the preventive benefits.

What to Expect From Your Wellness Visit at Coordinated Health

At Coordinated Health, our primary care physicians work with you to design a plan to help prevent disease and disability based on your current health and risk factors. The exam generally takes about 30 minutes. During your first annual wellness visit,  you can expect your physician to:

  • Check your height, weight and blood pressure
  • Review your medical and family history
  • Review your health risk assessment
  • Screen for cognitive impairment
  • Create a screening schedule for the next five years

AWVs after your first visit may be different. At subsequent AWVs, your doctor should:

  • Check your weight and blood pressure
  • Update the health risk assessment you completed
  • Update your medical and family history
  • Update your list of current medical providers and suppliers
  • Update your written screening schedule
  • Screen for cognitive impairment
  • Provide health advice and referrals to health education and/or preventive counseling services
