What should I wear?
For your comfort during exercise sessions, we recommend comfortable athletic shoes and loose-fitting exercise clothes.
How long are therapy visits?
Therapy visits last one hour.
What forms do I need to fill out?
New patients should arrive 15 minutes before appointment time to complete Patient Information, Therapy Benefit, Treatment Agreement and Policies Acknowledgement forms.
How will my treatment plan be personalized?
Our team works with you to best evaluate your injury or condition, to determine your goals and needs and to develop a personalized treatment plan.
What treatment schedule will I have?
The length of your treatment plan and frequency of visits is based on your specific injury or condition, your physician’s prescription, your insurance plan, and your therapist’s recommendations.
Will my therapy visits be supervised?
Yes. During each visit, you will be supervised and instructed on your exercises. Your therapist is available throughout each treatment session to answer any questions. In most cases, you participate in therapy several times a week for one hour per visit.
Will I be instructed about home exercise?
Yes. Your therapist will develop a home exercise program specifically designed for you, to complement the care you receive while with Coordinated Health professionals.
Is my doctor updated on my therapy?
Your therapist is responsible for maintaining regular communication with your doctor to provide updates on your progress.
